In order for us to provide an estimate of costs, your physical therapist will need to complete an evaluation to determine what treatments are appropriate.
Your physical therapist will review your medical history and perform a physical evaluation. Depending on your condition, your therapist may evaluate your strength, range of motion/flexibility, endurance, balance, coordination, posture, walking, stair negotiation, and body mechanics.
Please wear appropriate clothing (i.e. t-shirt, shorts, tennis shoes, sports-bra). Your therapist will then discuss with you their findings and suggested treatment plan.
You and your therapist will develop goals for physical therapy together. You will most likely be given a home exercise program that includes specific pictures and instructions for exercises.
Hammer Rehab and Fitness is contracted with most major insurance networks and carriers, including Medicare, Medicaid/Badger Care, and Workers Compensation. Our front office staff is more than happy to assist you to verify therapy benefits through your insurance carrier prior to scheduling so you are aware of number of visits allowed, co-pay or co-insurance, and/or limitations within your policy.
Yes. We will fax your initial evaluation to your physician. We also re-assess your condition as required by your physician and write a progress note documenting your progress. Please let us know when your follow up appointment is with your physician, and we will prepare a progress note to keep them up to date on your current status.
Your first appointment will last approximately 60 minutes. Your following appointments will last approximately 45 to 60 minutes depending on your condition and treatments.
No. Wisconsin is a "direct access" state and you do not need a prescription from your physician to receive out-patient physical therapy.
However, some insurance plans do require you to have a prescription from your physician in order for them to pay for physical therapy services.

Our therapists are highly trained health care professionals and are well qualified to evaluate a patient's condition, assess his or her therapy needs and, if appropriate, safely and effectively treat the patient. Therapists recognize when it is appropriate to refer patients to other health care professionals for consultation if needed.
